Blepharoplasty or midface lift

Question description

One plastic. Surgeon recommended lowereyelid surgery +midfacelift. When I said I was concern about entropion; advise was i can just go with transconjuctival without midface lift. Occuplastic surgeon advised transconjunctival transposition of fat and also skin removal 5mm. I am confused which one is the best. I do want to fix my under eye as I am hating it.

You have moderate lower lid fat collections and a tear-trough extending down and out over your upper cheek. The photo does not show your full face. My advice is that an open lower lid blephoroplasty with full mobilization of the orbital fat to a position well down into the cheek will give the best result. This allows a good pocket in the cheek to be made and good fixation of the fat to be achieved. Doing this adequately through a transconjunctival approach is far less predictable. The risk of ectropion (rolling out of your lid margin) should be low if minimal skin is excised (and you don't need a lot of skin excised). John

That is a great question and depends on the philosophy of your surgeon. I will start by saying that either procedure lower blepharoplasty or midface lift will rejuvenate the eyelid area. A blepharoplasty will target the area under the eyelids and hollowing and bags. A midface lift will lift the entire mid face including cheek and eye area. In my hands if your main concern is eyelids then I would offer a lower eyelid blepharoplasty to smooth and brighten your eyelids. If you are concerned with the cheek, naso-labial fold also then the discussion becomes a High SMAS Facelift that will lift the eyes, cheek and face.
