EXPLANT OF 14 YR. OLD TEXTURED IMPLANTS , UNDER MUSCLE. SHOULD THEY BE REMOVED IN ONE PIECE SINCE TEXTURED, OR SHOULD THEY BE STABBED, AND ANY REMAINING SCRATCH OUT?
Whether they can be removed in one piece will depend on whether the implant has ruptured or not. If it is not ruptured, it is easier to remove it in one piece. Ideally you would have an en-bloc (one-piece) removal if possible.
To do this, a larger skin incision than the routine for a breast implant insertion is needed and the scar capsule itself needs to be thick/strong enough. If the capsule is thin, dissecting it out can be difficult to impossible
The most difficult part of the capsule removal in a subpectoral implant is removing it from the chest wall. Often it is very attached to the ribs and cartilages. Once again removing it en-bloc can be impossible.
If some capsule can't be removed, the options are curetting it and diathermying (burning) it.
